Individualized Health Care Plan / Emergency Care Plan: You will find that you can print a copy of your child's existing Individualized Health Care Plan or Emergency Care Plan for bringing with you to your Health Care Practioner for updating for the next school year, through your Family Access on the district website or, you can print a blank copy from the link below.
Medication In School: White River School District #416 Board Policy No. 3416 addresses Medication at School. This policy is in accordance with Washington State Law and specifies the conditions by which medications are allowed to be given to students. The required "Physicians Orders for Medication in School" can be found in the Health Services Student Forms links below.
Student Immunizations:
White River School District #416 Board Policy No. 3413 addresses Student Immunizations. This policy is in accordance with Washington State Law and addresses information specific to immunization requirements. The Washington Department of Health Certificate of Immunization Status, required on enrollment and referenced in the district's board policy can be found in the Health Services Student Forms links below.
